certificate you Do Not need to fill out the parent(s) section.
Parent(s) name are not on our certificate of birth. However you can leave your first name for emailing purposes.
*If you are requesting a life certificate you Must fill in the parent(s) section excatly how
you want it to appear on the certificate. The parent(s) name(s) are put on our certificate of life.
